K weighted knobs come close, but pistol grips just feel fantastic.

The idea is that most people would carve their own and fit it to the default handle.

yes to both.
also my palm is on top of the knob when I shift, so that knob wouldn't be comfortable for me. (if it was a sequential shifter it'd probably be really nice though)

It's a really common old-muscle sort of thing, and they're actually absurdly comfortable, and supposedly faster than a traditional knob.
I wonder if Chrysler/Hurst made any RHD versions of the shift knob for export markets...

hmm, I guess I'll have to try one before I really have an opinion, with a short throw box it probably would be more accurate than a ball.
I'm sure they would have been some made locally, but I don't think they were ever popular in the muscle car scene here. I've seen a couple for drag setups with hand clutches
